Br3akfree Statement of Purpose (Outreach Day Centre Service)

- Br3akfree is built on a philosophy of supporting individuals to be the best that they can be, by developing each individual potential.
- The service brings a new and exciting approach with opportunities to stimulate and interest each individual in a friendly and comfortable environment, and quality support. We aim to provide a range of meaningful activities designed around the needs and preferences of each user, both on an individual basis, and in small group settings, at our base and in the community.
- We provide combinations of structured and flexible programmes, to suit the needs and choices of our customers.
- Br3akfree is a community based resource situated in Barking. Our resource is self-contained over 3 floors, but is wheelchair accessible to the ground floor. The location, close to the Town Centre, is ideal for users to access and enjoy the local scene and to be an integral part of the community. Our service users experience and participate in the active and bustling daily life of the area, while pursuing individual interests, thereby providing opportunities to develop new skills and interests.

We recognise that individuals do not necessarily fit into standard models and structures, and therefore we aim to operate flexibly, by offering services on a 7 day weekly basis, enabling individuals to enjoy our facilities and services on a full time, and part time basis throughout the year.

Br3akfree offers opportunities for learning and developing each individuals potential through general and specific experiences. Through our acceptance of the individual as a total person with potential to learn from every experience we will aim to provide as wide a range of activities as possible. However more specific opportunities for development are through implementation of our standards.

We pride ourselves in working towards outcome focussed quality standards, and ensure that staff are aware, and are committed to promoting these standards for people who use our services. We develop support plans, based on the needs and potential of each service user, to identify and set achievable and measurable outcomes. Our standards are at the heart of the support we provide, and are designed to enable our service users to:

Daily living and Occupational Skills
Br3akfree will provide support and opportunities for users to acquire new skills in daily living activities, to aid independence. These are activities such as cooking, baking, literacy and numeracy, handling money, shopping and use of multi-media. Staff will work with professionals and carers to support individuals to access and participate in more formal training, as well as supporting continuity in personal care and hygiene routines, including grooming, chiropody, pedicures and manicures.
We will support individuals to access work placements and employment opportunities where appropriate, through provision of the necessary practical assistance and by linking in with mainstream, as well as private and voluntary Employer and Training Organisations.
Emotional Support
Staff are trained to provide emotional support where required. We provide opportunities for our service users to communicate the need for this, and encourage and support each person in receiving the appropriate support and means of delivery.
Sensory and Physical Stimulation
Br3akfree offers sensory stimulation, suited to the needs and choice of the user, and based on professional assessment and evaluation. Offering the latest in multi-sensory environment, Br3akfree aims to offer each user a stress free relaxing and stimulating experience.
We have established relationships with mainstream and private &voluntary health focussed organisations, and support our service users to access and benefit from these resources.
Music Therapy
We have a regular music therapist for Individual and group sessions using a range of skills and instruments. Our service users also participate in community dance sessions and events.
A range of Arts and Crafts activities are offered to assist and support users with creativity and expression as well as producing outcomes.
Community Access
Our focus is on community-based activities with emphasis on local and wider community involvement and participation. Br3akfree has established links with mainstream and private and voluntary agencies to enable access to sporting and recreation facilities within the borough. Staff will support service users to participate in guided walks, as well as enjoying cycling, sightseeing, visiting places of interests, participating in other day-to-day activities that are part of our everyday life. Br3akfree aims to provide and promote opportunities for all our users to enjoy social events and activities during day and evening by providing escorting, supporting and transporting services.

Our staff are one of our biggest assets and therefore we will ensure through recruitment and training, that staff have the appropriate attitudes and values to support all our customers and users. We strive to ensure that the ethos and culture of our facilities are inclusive so that all users are integrated into the excellent facilities and services we offer. Staff have wide ranging experience, encompassing all aspects of care and support.

We aim to keep individuals who use our facilities, at the centre of our focus and to cater for the individuals needs and choices, in every area of our provision. Br3akfree is a resource designed to offer a complete person-centred, fulfilling experience, which is designed to provide new options, as well as widen existing choices available to our service users. We will strive to ensure that the individual has an exciting and meaningful experience with every visit to our facility, and with each interaction with our services.
Our services are designed to allow the users to experience the rich diversity of the local environments, while facilitating and supporting access to a range of activities that may have been previously off limits. In pursuing these outcomes, we are committed to working in partnership with families and other agencies to provide individually designed packages of care and support, to provide and maintain continuity of essential routines and aspects of care, to give carers peace of mind. -

This Statement of Purpose has been produced in accordance with the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010, and the Care Quality Commission (Registration) Regulation 2009. The document outlines the services that would be provided for the regulated activity of Personal Care, for our prospective service users, their relatives, friends, carers and their representatives / advocates.
It sets out the following:

- The range of qualifications of our staff.
- The compliments and complaints policy.

Br3akfree Limited is a privately owned community based resource situated in Barking. Br3akfree was originally established in November 2007 as a Day / Outreach Centre for Adults with Learning Disabilities. This means that our activities are mostly community based, and it is still providing only these services. Our resource is self-contained over three floors, but is wheelchair accessible to the ground floor. The location, right in the Town Centre, is ideal for service users to access and enjoy the local scene and to be an integral part of the community. Our service users experience and participate in the active and bustling daily life of the area, while pursuing individual interests, thereby providing opportunities to develop new skills and interests.

Thus in summary, the thrust of our service provision i.e. our objectives, include the following:1. To focus on service users

3. To ensure service users are provided with information concerning the services provided, and any changes to those services which may affect their well-being.
4. To meet service users needs by ensuring that new service users needs and preferences are thoroughly assessed prior to the commencement of their service.
5. To ensure that service users needs are regularly reviewed and updated to reflect their changing needs.
6. To provide personal care and support in ways which have positive outcomes for service users and to ensure that service users have access to personal files.
7. To encourage service users and their carers to participate as fully as possible in formulating their care plans.
8. To enable and support service users to retain their independence of thought and activity.
9. To meet the service user physical and emotional needs in a non-judgemental way.
10. To ensure that the forms of communication, language and address used are suitable for the needs of the service user.
11. To ensure that all information received in the course of carrying out our duties is treated with the utmost confidentiality and is not passed to any unauthorised persons; and service user records will be accurate and fit for purpose.

13. To ensure that all health and safety procedures are followed at all times.
14. To ensure continuous service improvement via staff performance management, and learning from information / feedback from customer satisfaction surveys; and our Complaints / Compliments policy and procedure.
15. To recognize the service user diversity ensuring religious, cultural, racial and gender identities are respected and to request service users and their carers respect the rights of staff to the same non-discriminatory treatment.

Registration with the Care Quality Commission would enable us to provide services under the following regulated activities as defined in the Health and Social Care Act 2008:

Br3akfree Domiciliary service provision would offer a range of home care services, tailored to meet service users individual needs. We will ensure that our care workers provide receipts and keep a record for all services which involve financial transactions. The services that we will provide are:
Meal Preparation
Some service users are unable to or / and do not have the opportunity to eat quality meals. In consultation and agreement with the service user, our care workers will prepare nutritious food for them, taking into consideration any dietary, cultural and religious needs. This service will ensure that service users have the well balanced diet that they need for healthy living.
Feeding
Our care workers are able to assist service users with their eating and drinking needs. We understand that it is sometimes difficult for an adult to accept that they have to be fed, and that the sense of loss of independence is real. Our care workers understand the feelings this can cause and handle this with sensitivity by taking extra care to make mealtimes special through interaction.
Light Housekeeping
This service includes sweeping, vacuuming, dusting and mopping floors; cleaning bathrooms to include sinks, showers, baths and toilet, cleaning kitchens, sinks, appliances, counters, taking out the rubbish, tidying all rooms, cleaning any interior windows that can be reached without steps. Br3akfree can also assist with the provision of outdoor cleaning and/or maintenance e.g. gardening, by utilising vetted third party suppliers.
Grocery Shopping
Based on the service users choices and instructions, our Care Workers can shop with or without being accompanied by the service user.
Laundry & LinenWashing
These services can be provided in either the service user’s home or the Launderette, and includes washing, drying, ironing and putting things away, utilising the service users preferences at all times.
Running Errands
Care workers will assist with picking up prescriptions, going to the post office, or other routine chores. This service may be provided with or without the service users present.
Clothes Shopping
Our care worker will do shopping for the service users or take the service users to their favourite shops as required. While they are out, they can accompany the service users on a stroll through a shopping centre, then lunch with friends, or undertake other recreational activities.
Recreational Activities
Whether its cards, board games, or a walk in the park, going to Bingo clubs, bowling or tea parties, our care workers can take the service users out to enjoy their favourite activities, and thus help keep our service users stay active in mind and body.
Bathing
Bathing is essential to an individuals hygiene and sense of well-being. It rejuvenates body and soul, provides relaxation while cleansing the skin and stimulating circulation. Our care workers can assist with a shower, bath or bed bath depending on the service users need.
Oral Hygiene
Proper mouth, teeth, denture and gum care are essential to good oral and dental hygiene. Our care workers can assist service users with daily oral care, in order to prevent tooth decay and gum disease which can cause bad breath.Toileting
This is an important and normal body function that is private and not openly discussed. Individuals who require help and support to carry out this function are often uncomfortable and embarrassed about this loss of their independence. Our care workers will provide an acceptable, comfortable, and safe environment for all types of toileting needs to respect service users privacy and dignity.
Incontinence Care
The changing of adult pads, and emptying catheters are all duties that our care workers can provide. The inability to control urination or defecation is embarrassing to individuals and can become unhygienic if not taken care of correctly. Our care workers understand that this should be handled with sensitivity, in order to maintain the dignity and respect of the service user.
Grooming & Dressing
We will ensure that our service users are appropriately dressed and feel positive and confident about their appearance. Care Workers will help to wash & comb hair, ensure they wear clean clothes, do and undo buttons, put on socks, wear shoes, tie shoe laces, and assist service users with routine activities of daily living.
Medication Assistance
Our carers may only administer prescribed medications. They can assist the individual who shares in the responsibility of taking their own medication. Details of medication will be recorded during the assessment visit but it is important that medications are either provided in blister packs or are kept under review regularly with the service user or family member. We will record when medication is given or when it is observed being taken.
Mobility
Activity and exercise are vital to a healthy lifestyle. Our carers may assist the service users with all types of mobility and passive range of motion exercises e.g. going on gentle walks. The benefits of movement to both physical and mental health are seen in everyday life. Benefits include increasing stamina, strengthening the heart, lowering blood pressure, improving digestion and sleep. In addition it improves mood, releases stress and increases mental alertness
Organising Incoming Post
We can assist service users in separating the junk mail from the important mail, and let them know when a response to a letter is needed or bills are due. We can assist with sending out cards, letters, and packages to the service users family and friends.
Companionship / Sitter Services
Whether the service users needs conversation, company, or just eating a meal together, having someone to visit can help make for a happier, healthier life. Interacting with every service users is of utmost importance.
Periodic Reviews with Family
Apart from their annual reviews, we can contact the family as and when required based on the changing needs of service users to discuss their welfare and progress, address any concerns, and review and update their care plans. This is useful for service users whose families live far away or may not be in daily contact with their loved ones. Communication in this instance may occur via phone, e-mail, or any other method the service users and their family prefers.
Transportation
Transportation may be provided to service users in addition to other services being provided. Our drivers all have the appropriate PSV driving licenses given that we currently pick-up and drop off our service users to and from our day centre. Care workers may provide trips to doctor or hospital appointments, barbers/hairdressers, chiropodists, church services, shopping, or wherever else the service users would like to go. Getting senior citizens out and about is good for their physical and mental alertness
Keeping in Touch service
This is a telephone service for those service users who may not need someone coming to their home every day, but they and their families will have peace of mind knowing that if they need something, they can call us for assistance and support. We can also call from time to time to check if the service user is alright. The calls can be made as often as the service users want. It could be once a week or three times per day. A care worker or an office employee may make the calls. The goal is to build the relationship on the phone.
Other Specialist Services
From time to time care workers may be asked to undertake some tasks, which may be considered to be specialist. These tasks may be undertaken following appropriate risk assessments and only after specific training. The care worker will be trained in the procedure before undertaking the tasks with the person with care needs by a trainer with a relevant qualification, e.g. occupational therapist, speech therapist, or nurse.
Such tasks may include:
1. Assisting with artificial feeding
2. Catheter care changing bags, monitoring output
3. Assistance with eye or ear drops -
Our care workers will not undertake tasks that require the skills and expertise of clinical professionals. Such tasks include:
1. Toe nail cutting
2. Ear syringing
3. Removing or replacing urinary catheters
4. Bowel evacuations
5. Bladder washouts
6. Injections involving assembling syringes, administering intravenously, controlled drugs
7. Filling of oxygen cylinders
8. Lifting from the floor unaided
9. Tracheotomy care changing tubes
Br3akfree will not provide nursing care services other than those already referred to. -

Range of qualifications of our current care staff
Our staff are one of our biggest assets and therefore we ensure through recruitment and training, that staff have the appropriate skills, attitudes and values to support all our customers and service users. This is to ensure that only support staff that are passionate about providing a quality service are engaged.
Once they are employed they undergo induction training for one week, to make sure they understand the principles of care and the companys ethos. They then attend mandatory training covering a range of courses. For instance, staff are trained in Moving and Handling and in First Aid, Safeguarding Vulnerable Adults etc and staff involved in food preparation are trained in Food Hygiene. Staff also attend refresher courses to enhance their work performance and to keep abreast of changes. The minimum qualification requirement for support staff at Br3akfree is NVQ2. Those who do not have this qualification are required and encouraged to take it. Employees with NVQ level 2 are encouraged to continue with their training and assessment in order for them to gain a successful outcome with Level 3, and a number of our staff have succeeded in achieving this qualification. NVQ4 is the minimum requirement for Managers.

Staff understand that they are responsible for maintaining their health and safety and that of others at work. This is discussed during team meetings and during supervision. They have all received Health and Safety training in line with the Health and Safety at Work Legislation, covering Fire, Manual Handling, CoSHH and DSE etc. They also receive epi-pen training, epilepsy management and administration of medication.
All our staff are DBS checked and part of their induction training is to do a full week of shadowing other staff to help them to gain an understanding of the care needs of our service users as per their risk assessment, including Health and safety risk assessments. This ensures that staff working alone with service users are safe and able to support the service users effectively.
We subscribe to Quality Care Systems (CQS), a subsidiary of CQC, for all our policies and procedures, including Health & Safety, Risk Assessment and Safe Guarding policies and procedures. This also ensures that the Health & Safety Policy is updated annually in line with legislation.
We have contracted a Health & Safety Consultancy firm to cover all our Health & Safety requirements. They provide staff Health & Safety Training, and carry out Health and Safety Audits / Risk Assessment and Inspection. The regular Health and safety Audits are to ensure that premises and other facilities are fit for purpose, with any defects identified immediately rectified. The risk assessment also cover risks associated with lone working for staff who support users alone either at the day centre or in the community.Access to Services
